Dear Sir,

I am Atul Wason, a regular visitor of the library for the past 6 years. I am sorry to inform you that I have misplaced my Library Identity Card. 

I request you to kindly issue a Duplicate Identity Card for the time being until I find the original one. If I fail to find the original card I will apply for the new card. For the time being, I request you to issue a Duplicate Identity Card as my final exams are approaching and I need reference books. 

I am enclosing a copy of the yearly membership slip for your reference. Kindly issue a Duplicate Identity Card at the earliest.
